ELIGIBILTY:

  • To be eligible you must be 18-70 years old and working a minimum of 20 hours a week.
  • Children under the age of 26 are eligible regardless of marital or dependency status.

 

PREMIUMS:

  • The cost of this product is less than $.75 per day for an employee and less than $2.00 a day for a family plan.
